Airway Management（気道管理）

G2000までのACLSコースでは挿管の実習もありました。が、2日間コースのタイトな時間の中で気管内挿管の技術を習得するのは非常に難しいこと、G2005では質の高いCPRには必ずしも気管内挿管は必要でないこと（BVMによる十分な換気ができていればよい。）などからACLSコースでの気管内挿管のスキルブースはなくなりました。また、シナリオステーションでも挿管は練習しないし、指示しなくてもよいことになっています。ただし、高度な気道確保器具については十分な理解が必要で、気管内挿管されている状態での換気と胸骨圧迫の練習はありますよ。
とにかく重要なことは、きちんと患者さんの胸郭が挙上するだけの換気ができているということと、過換気を避けるということです。



気道確保 ⇒ 頭部後屈、顎先挙上（外傷があれば下顎挙上）
頭部後屈顎先挙上で、十分に気道確保ができなければ、エアウェイによる気道確保を選択します。
エアウェイ ⇒ 口咽頭エアウェイ（OPA）
　　　　　　　　　鼻咽頭エアウェイ（NPA）

《口咽頭エアウェイ（OPA）》
舌が咽頭後壁に落ち込まないようにする。
　　　　咽頭の吸引を容易にし、気道管理用具を噛むことをことを防ぐ。
　　　　咳・嘔吐反射のない、反応のない患者に使用する。
　　　　口に入れるときは反転した状態で挿入する。
　　　　もしくは代替法で舌圧子または類似の器具で舌を
　　　　口腔底に押しつけながら反転せずに挿入する。
　　　　サイズ：成人8～10号
　　　　サイジング：口角～下顎角

《鼻咽頭エアウェイ（NPA）》
　　　　意識があり、咳・嘔吐反射のある患者でも使用できる。
　　　　重症の頭部、顔面の外傷の患者には注意して使用する。
　　　　サイズ：鼻穴、患者の小指の太さ
　　　　サイジング：鼻穴～耳たぶ
　　＊ Airway挿入後は必ず呼吸の確認をすること！！



《酸素投与》
room airの酸素濃度はどのくらいでしょう？？
鼻カニューレでは酸素流量を1L増やすと吸気中酸素濃度（FiO2:エフアイオーツー）はどのくらい増えるのでしょう。
あ、ここで注意点を一つ。これはあくまでも酸素流量(L/分)に対するFiO2の期待値であって、極端な話患者さんが口呼吸をしていたら酸素濃度は上がりませんよね。
一般に鼻カニューレでの流量を1L増やすとFiO2は4％上がるといわれています。
room airの酸素濃度を計算しやすいように20％と考えると
　　1L = 24％
　　2L = 28％
　　3L = 32％
　　4L = 36％
　　5L = 40％
　　6L = 44％・・・・・となります。鼻からの酸素投与は教科書的には6Lが限界ですから、まあ空気中の倍の濃度くらいはいけるということになりますね。
鼻カニューレで患者さんに酸素を投与するのは患者さんの状態がいい時。SpO2 = 95％くらいと考えてください。
では、もっとSpO2が低い患者さんにはどうするのでしょう？　・・・・・・・そうです。フェイスマスクでの投与ですね。
リザーバーがないフェイスマスクでの酸素投与は6L／分でFiO2 = 40％、7Lで50％、8Lで60％くらいと言われています。
リザーバーつきマスクでは9Lで90％、10～15Lでほぼ100％のFiO2を期待することができます。
他にベンチマスク（ベンチェリーマスク）、インスピロンなどがありますがあまり一般的ではないのでここでは省略します。・・・・コースでも使わないしね。


おまけ
挿管中の患者の状態が悪化した場合は、どうしましょう。
ACLSの考え方の基本は生命維持がきちんとできること。そして専門医に引き継ぐまで、患者に害にならない科学的根拠に基づいた標準的な処置ができることなのです。それなのに、挿管中の患者の状態が悪化たりしてはいけません。死につながる可能性のあることをやってはいけないのです。ですから、処置の前の評価と、処置のあとの確認は必ず必要です。
まずは、次の可能性を考慮しましょう（DOPE）。
　　Ｄ：位置異常（Displacement of the tube from the trachea）
　　Ｏ：チューブの閉塞（Obstruction of the tube）
　　Ｐ：気胸（Pneumothorax）
　　Ｅ：機器不良（Equipment failure）




《補助換気》
しっかりした脈が触れる患者さん。つまり、心拍出があり循環が保たれている患者さんで呼吸が十分でない場合はBVMによる補助換気が必要です。
ここでも重要なことは過換気を避けること。酸素流量15L／分でリザーバーつきBVMを使うと、100％の酸素供給ができます。
補助換気は5～6秒に1回、つまり1分間に10～12回です。2分間実施して再評価します。

気道管理のスキルブースでは補助換気の一人法、二人法、輪状軟骨圧迫を練習します。
